Immunization Requirements
Parents need to be aware that if documentation is not received at the school by the first day of school, the school district is required to exclude the student from participation in school. Failure to do so will result in your child not being able to attend school.
Requirements for all School Students in K-12

4 doses of tetanus (1 dose on or after the 4th birthday)

4 doses of diphtheria (1 dose on or after the 4th birthday)

4 doses of acellular pertussis (usually given as DTP or DTaP -- 1 dose on or after the 4th birthday)

4 doses of polio (4th dose on or after the 4th birthday and at least 6 months after previous dose given)

2 doses of measles (usually given as MMR)

2 doses of mumps (usually given as MMR)

2 doses of rubella (usually given as MMR)

3 doses of hepatitis B (an additional dose would be necessary if the first three are not administered within the proper time frame)

2 doses of varicella (chicken pox) or written statement from physician/designee indicating month and year of disease or serologic proof of immunity
Requirements for all students in grades 7 on the first day of 7th grade (in addition to the above vaccines)

1 dose of tetanus/diphtheria/pertussis (Tdap)

1 dose of meningitis vaccine (MCV4)
Requirements for all students in grade 12 on the first day of 12th grade (in addition to the above vaccines)
- An additional dose of meningitis vaccine (MCV4) received after the age of 16 (for a total of 2 doses).
