Form 1095-C Requests

Form 1095-C for 2024 is available upon request for any BPSD employee offered health insurance coverage through the district. The form provides information on the health coverage offered to an employee by the district for the previous calendar year. All employees who were offered health insurance, even those not currently enrolled, may request a form.

Requested forms will be provided within 30 days after the date of request. All requests and questions should be directed to Julia Herrman in the Business Office. Contact details are provided below:
