State Grade Level Requirements

ANNUAL PA STATE/DISTRICT REQUIREMENTS BY GRADE LEVEL
Kindergarten
1. Kindergarten Registration Form
2. Copy of Official Immunization Record
3. Physical Exam Form by a private physician
4. Dental exam by a private dentist
 
Grade 1
1. Returning Student Registration Form (found in the HAC)
2. Physical Exam Form by a private physician (if not submitted during Kindergarten)
3. Dental exam by a private dentist (if not submitted during Kindergarten)
 
Grade 2
1. Returning Student Registration Form
2. Dental exam by a private dentist during Grades 2 or 3
 
Grade 3
1. Returning Student Registration Form
2. Dental exam by a private dentist (if not submitted during Grade 2)
 
Grade 4
 
Grade 5 1. Returning Student Registration Form
2. Physical Exam Form by a private physician during Grades 5 or 6

Grade 6
1. Returning Student Registration Form
2. Physical Exam Form by a private physician (if not submitted during Grade 5)
3. Dental exam by a private dentist during Grades 6 or 7
4. Scoliosis screening conducted by the school nurse
 
Grade 7
1. Returning Student Registration Form
2. Updated copy of immunization records for Tdap/MCV 3 Dental exam by a private dentist (if not submitted during Grade 6)
4. Scoliosis screening conducted by the school nurse
 
Grades 8 & 9
 
Grades 10 & 11 1. Returning Student Registration Form
2. Physical Exam Form by a private physician during grades 10 or 11

Grade 12
1. Returning Student Registration Form
2. Updated copy of immunizations for MCV
 
New Students
1. New Student Registration Form
2. Copy of Official Immunization Record (see immunizations)
3. Must provide a record of previous dental and physical exams on entry and/or required at appropriate grade levels
 
STATE-MANDATED HEALTH SCREENINGS
Pennsylvania Health Law mandates that all students have complete immunization records upon entering school (at any grade). State-mandated screenings are completed in all grades.

These screenings are performed by the certified school nurse and are not to be substituted for regular check-ups with your healthcare provider. You will be notified by mail with a referral form if your child has not met the guidelines for the screenings as set by the Pennsylvania Department of Health.

The referral form is to be completed by your private healthcare provider and returned to the health office at your child's school. If you choose to have your child exempt from these mandates please notify your school nurse and provide documentation from your child's private physician with the required mandated information. If you would prefer to have your child seen by the school physician or dentist please let your school nurse know by November 1 of the current school year.

Growth Screening:
Height, Weight, and BMI (Body Mass Index) - are performed annually in Grades K-12. You will receive the results by mail each year, which will include the Body Mass Percentile which is used as a guideline to help assess whether a person may be overweight or underweight. (Visit the AAP (American Academy of Pediatrics for more information on BMI.)

Vision Screening:
Held annually in Grades K-12. This screening is not a substitute for a comprehensive eye examination by an eye care specialist. If your child fails this screening, a referral will be sent home.

Hearing Screening:
Performed annually in Grades K, 1, 2, 3, 7, and 11. This screening is not a substitute for an audiology examination by your healthcare provider. If your child fails this screening, a referral will be sent home.

Scoliosis Screening:
Conducted in Grades 6 and 7. The purpose of the screening is to detect any abnormal curvature of the spine. If your child fails this screening, a referral will be sent home.
