Sports Physical Form
Effective October 1, 2007 the PIAA has changed its requirements for student physicals.  Students are now only required to have one full physical per school year, BUT parents must complete the Recertification for Participation form and the Student Information Sheet prior to the beginning of each sports season the student participates in during the current school year.  Athletes who have not had a physical after June 1, 2007 will need to complete the entire physical packet.  All of the forms have been revised from what was previously posted, so please click on the above link to access the most up-to-date forms.  If you have questions, please call the Athletic Office at 412-854-8548.
