immunization requirements

Please click here to read a letter, signed by Nurse Department Facilitator Tracy L. Ford, MSN, CRNP, that is similar to a letter that will be sent home to all Bethel Park parents from their child's school nurse regarding the new immunization requirements that will be in effect beginning the 2008-2009 school year.  The information below now reflects those new requirements.

 

Parents need to be aware that if documentation is not received at the school by August 31, 2008, the school district is required to exclude the student from participation in school.

 

Requirements for all School Students in K-12 effective with the beginning of the 2008-2009 school year

  • 4 doses of tetanus (1 dose after the 4th birthday); 3 doses if the series started after 7 years of age

  • 4 doses of diphtheria (1 dose after the 4th birthday); 3 doses if the series started after 7 years of age

  • 3 doses of polio

  • 2 doses of measles

  • 2 doses of mumps

  • 1 dose of rubella

  • 3 doses of hepatitis B

  • 2 doses of varicella or written statement from physician/designee indicating month and year of disease or serologic proof of immunity

Requirements for all students in grades 7-12 (in addition to the above vaccines), effective with the beginning of the 2008-2009 school year

  • 1 dose of tetanus/diphtheria/pertussis (Tdap)

  • 1 dose of meningitis vaccine (MCV4)

Provisional Enrollment

If a child has not received all of the vaccines required, the child may be provisionally admitted to school only if evidence of the administration of at least one dose of each vaccine is given to the school administrator or designee and the parent or guardian's plan for completion of the required immunizations is made part of the child's health record.  Immunization requirements should be completed within eight months of the date of provisional admission to school.
